You can register by completing a registration form (GSM1). When you register, we will ask you to make an appointment to see the practice nurse (or doctor if you are on regular medication) for a new patient health check.
** Due to the restrictions in place in dealing with Covid-19, please complete each of these forms and submit to the surgery by e-mail to info.mydoctors@nhs.net
You will need to complete each form and e-mail it to us along with your photo ID. We are not currently offering routine checks for new patients, but will do so once this crisis is past.You will receive all other information via e-mail regardign your registration and we look forward to seeing you at some point!
